All Incident Reports must be made by a Club Committee Member

Reporting an Incident

When raising an incident report against another club, please follow this process.


  1. Collate all information required.  Detailed version of events, including witness statements and supporting documentation.
  2. Discuss an action plan with your club CWO or Committee Member.
  3. CWO to contact CWO of accused club to discuss incident and seek resolution. 
  4. Communication if not in writing should be followed up in writing to confirm (via email).  
  5. All email communication between clubs should cc report@fdyfl.com 
  6. It is the responsibility of clubs to find resolution.
  7. The League will oversee reports and collate this information.
  8. The League will take action when required.  Repeat offenders will be investigated. 
  9. It is the primary responsibility of clubs to manage the behaviour of their members and volunteers.
